**About this role**
As a **Care Partner**, you'll be the vital link between our clients and the new _Support at Home_ program—ensuring older Australians receive the right care, at the right time, in the way that best suits them.
This is a dynamic, hands-on leadership role where you'll guide a team of passionate Home Care Workers to deliver high-quality, person-centered care that upholds dignity, independence, and quality of life. You'll work closely with clients, their families, and external providers to tailor support and foster strong, trusted relationships.
**Key responsibilities**
- ** Assess Client Needs**: Conduct initial and ongoing assessments to understand physical, emotional, and social needs, identify risks, and tailor care plans.
- ** Care Planning**: Develop and update person-centered care plans aligned with client goals and Support at Home funding.
- ** Coordinate Services**: Organise in-home supports and liaise with external providers to ensure timely, quality care.
- ** Advocate and Educate**: Support clients to understand their funding, choices, and rights, and advocate for access to appropriate services.
- ** Budget Oversight**: Monitor individual budgets and guide clients on effective use of funds.
- ** Relationship Building**: Build strong connections with clients, carers, and providers.
- ** Compliance and Safety**: Maintain accurate documentation, uphold aged care standards, and proactively manage risks.
**What you can bring to the team**
You're a natural people leader with a heart for community care and a talent for getting the best out of your team. You're confident navigating the aged care space and thrive in a fast-paced, mobile work environment.
**To be successful, you'll need**:
- Cert III in Individual Support (Ageing) or equivalent;
- Experience leading a team in aged or community care;
- Working knowledge of aged care standards, funding, and compliance;
- Current First Aid and CPR certification;
- A reliable, comprehensively insured vehicle and a valid driver's licence;
- Strong communication, organisation and problem-solving skills.
Customer service or sales experience desirable but not essential.
**Possess or willingness to obtain**:
- A current National Police Certificate;
- Up-to-date First Aid and CPR Certification;
- Undergo a National Personal Insolvency Check.
