We are seeking a compassionate and skilled Support Practitioner to join our Children, Youth, and Families team in Caboolture.
The successful candidate will facilitate intake, screening, and placement of children and young people with foster and kinship care families. They will deliver assessments, crisis support, and therapeutic interventions as needed. The Support Practitioner will develop and evaluate care plans tailored to individual needs.
- Deliver evidence-based parenting programs and collaborate with community partners to support families.
- Recruit, assess, and support foster and kinship carers, including facilitating approval processes.
- Lead case management planning and maintain accurate client records.
- Provide flexible, in-home support services, including after-hours and on-call duties.
- A relevant degree in social work, psychology, or related field - or equivalent experience.
- A strong understanding of child protection, out-of-home care, and relevant legislation.
- Skill in care assessments and trauma-informed practice.
- Knowledge of challenges faced by Aboriginal and Torres Strait Islander communities.
- A current driver's licence, National Police Check, Working with Children (Blue Card) and Licensed Care Service (Child Protection).
