Monash Health is committed to providing safe and high-quality, person centred care. In our relentless pursuit of excellence we are seeking a highly motivated and dynamic health care practitioner to join the team in the Shared Decision Making Office to support:

- the ongoing development of a person-centred voluntary assisted dying model of care for the organisation
- consumers who enquire about or request voluntary assisted dying, to access the required specialist voluntary assisted dying eligibility assessments and care to support their end-of-life care preferences
- shared decision making practices to be embedded into routine care
- consumers in discussing and documenting future care preferences (advance care planning)

**Shared Decision Making Office**

The Shared Decision Making Office at Monash Health supports person-centred care, autonomy and self-determination. The Office has several key tasks and responsibilities aimed at supporting and embedding a sustainable approach to shared decision making and advance care planning across the organisation to promote an individual's autonomy in regard to future care planning and end-of-life care preferences including voluntary assisted dying.

The Office has responsibility for providing clinical and organisational support for the implementation of the Voluntary assisted dying Act (2017). Key tasks include the comprehensive psychosocial assessments and coordination of access to VAD eligibility assessments for those patients requesting access to voluntary assisted dying

**About The Role**

The _Voluntary Assisted Dying (VAD) Liaison Officer / Shared Decision Making Facilitator_ provides advanced clinical assessment skills and management experience to support high-quality patient centred care by providing and promoting shared decision making practices through:

- staff education, training and mentoring,
- facilitated advance care planning discussions with patients to document future care preferences
- comprehensive and collaborative end-of-life care and planning for patients requesting access to voluntary assisted dying as an end-of-life option

This position works closely with the Advisor, Shared Decision Making, the Director of Medical Services and the Chief Medical Officer to ensure the provision of safe and effective person-centred care.

This is permanent part-time role (72 hours per fortnight) located at Monash Medical Centre, however working across all Monash Health sites is required.

**What You Need**
- Normally have at least 10 years' post qualifying experience
- Completion of a post graduate qualification relevant to the position requirements
- Understanding of the _Medical treatment planning and Decisions Act (2016) and the Voluntary Assisted Dying Act (2017)_
- Current Victorian Drivers Licence essential
- Australian working rights
