Nepean Hospital is a teaching hospital that provides tertiary referral services in emergency care, coronary care, diagnostics, gynaecology, paediatric, neonatal intensive care/ICU, maternity, mental health, rehabilitation, and surgery. We are seeking a passionate and skilled Social Worker to join our Aged Care team at Nepean Hospital. This role involves delivering a high-quality social work service to patients across the Aged Care wards, drawing on a strong foundation of social work theories to guide practice. You will work collaboratively with multidisciplinary teams to ensure the best possible outcomes for patients, supporting efficient and effective care planning and delivery.
As a member of our Aged Care Social Work team, you will conduct psychosocial assessments, participate in case conferences, facilitate family meetings, and provide a range of counselling services. Your contribution will be vital to both case management and discharge planning, helping patients and families navigate complex health and social care needs with compassion and professionalism.

What You Will Bring To The Role
- Tertiary Qualification in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW)
- Demonstrated understanding of Aged Care Social Work within a Hospital environment
- Demonstrated high level communication, report writing and computer skills
- Demonstrated ability to work independently and as a part of a Multidisciplinary team.
- Demonstrated understanding of psychosocial stressors that could impact on the admission.
- Basic knowledge of My Aged Care, Carers Gateway, NSW Civil Administrative Tribunal - Guardianship Division, National Disability Insurance Scheme, the Ageing and Disability Commission and Residential Aged Care.
- Demonstrated capacity to Link theory and practice
- Current unrestricted driver's license and willingness to travel for work.
