About the Role

Pathway Links is seeking a dedicated and experienced NDIS Support Coordinator to join our dynamic team in Adelaide, SA. As an NDIS Support Coordinator, you will be responsible for assisting participants in the National Disability Insurance Scheme (NDIS) to access and coordinate their support services.

This is a part-time position, with the strong potential to transition to full-time within the coming months, based on client growth and organisational needs. This role is essential in helping our clients achieve their individual goals and live more independent lives.

What You'll Be Doing

  • Developing and implementing tailored support implementation plans aligned with participants' NDIS goals and aspirations.
  • Coordinating and monitoring the delivery of NDIS-funded supports and services to ensure effectiveness and continuity.
  • Supporting participants to exercise choice and control by building their capacity to navigate and manage their supports independently.
  • Liaising with service providers, health professionals, and community organisations to facilitate collaborative and integrated support.
  • Reviewing and updating support implementation plans regularly to reflect participants' evolving needs, preferences, and goals.
  • Preparing for plan reviews by gathering evidence, tracking progress, and ensuring services align with the participant's NDIS plan.
  • Maintaining accurate, up-to-date case notes and documentation in line with NDIS practice standards and reporting requirements.
  • Providing guidance and practical support to participants and their families/carers in understanding and accessing their funded supports.
  • Identifying and addressing any service delivery issues or gaps by working proactively with stakeholders to find appropriate solutions.
  • Ensuring all work is conducted in accordance with the NDIS Code of Conduct, organisational policies, and participant safeguarding practices.

What We're Looking For

  • Relevant education or certification in disability or a related field such as social work or community services
  • Minimum 2 years' experience working as an NDIS Support Coordinator or in a similar role
  • Excellent communication and interpersonal skills, with the ability to build rapport with clients
  • Strong problem-solving and advocacy skills, with a focus on achieving positive outcomes
  • Thorough understanding of the NDIS and relevant disability legislation and regulations
  • Proficient in using client management systems and technology to support your work
  • Demonstrated commitment to person-centred, ethical and professional practice
