Role Summary:

  • 12-month contract position
  • 38hrs per Week
  • S.C.H.A.D.S. Industry Award – Level 5, based on experience

We are on the lookout for an enthusiastic, compassionate, and highly organised Community Services professional to join our vibrant team. As a Disability Support Coordinator for our Home and Living Team, you will over-see and manage a team of Disability Support Workers to ensure the delivery of high-quality services to NDIS participants. These services should meet the participants' needs and expectations while aligning with organisational, service, and individual participant goals.

Responsibilities include (but are not limited to): 

  • Responsible for staff rostering to ensure adequate support for all activities, taking into consideration the needs and choices of participants
  • Work with Participants to achieve their goals in line with their NDIS plan
  • Work with Disability Support Workers and Participants to help build capacity and independence
  • Engage with Participants, their supports and third-party organisations in a professional manner
  • Ensure Home & Living services are within funding guidelines and the principals of the N.D.I.S.
  • Responsible for ensuring staff are equipped with information, instruction, resources and training.
  • Ensure Participant information is maintained and regularly reviewed including personal details, risk assessments, WHS checks, case notes and individual plans.
  • Able to assist in a crisis and be available for emergency response
  • Ensure essential supports and services are provided to participants as agreed.
  • Regularly visit site locations to ensure compliance and quality of service delivery.

To be successful in this role you will have:

  • An ability to engage with stakeholders and identify solutions
  • An ability to work independently, multi-task and time manage
  • Excellent communication and interpersonal skills.
  • Experience in monitoring budgets, client services, funding packages, and rostering.
  • Experience in providing support to Individuals via case management
  • Have relevant tertiary qualifications or certificate with relevant experience OR relevant extensive experience in Accommodation Coordination, Disability or Community Services.
  • Working with Children (Blue Card) and Disability Services (Yellow Card)
  • Right to Work within Australia
  • Current Queensland Drivers Licence,
  • Registered motor vehicle and comprehensive car insurance
