Uprety Home Care is seeking a dedicated
Clinical Coordinator (RN)
to join our growing community nursing team. This is a flexible part-time position ideal for an experienced nurse who's passionate about supporting others and improving clinical outcomes.

You'll oversee care planning, documentation quality, and training across our nursing and support teams, ensuring services are safe, consistent, and person-centred. This role includes developing and reviewing care plans, coaching nurses, maintaining governance registers, and providing clinical guidance to the Intake team.

Most of your work will be completed remotely, with monthly face-to-face meetings in Mount Kuring-Gai and occasional participant visits (with notice).

Hours:
Tuesday 7.5h, Wednesday 5h, Thursday 7.5h (some flexibility available).

What you'll need:

  • Current AHPRA RN registration
  • Experience in community or disability nursing
  • Strong documentation and mentoring skills
  • Driver's licence, Police Check, WWCC & NDIS clearance
