This is a maternity relief position and should the permanent incumbent require an early return from maternity leave, this contract of employment will cease with two weeks notice. Maternity relief positions can be for a period of up to 12 months, with the possibility of continued employment for up to a further 12 months for maternity leave reduced hours, therefore there are no claims to permanency.
Are you ready to lead change in aged care? Central Coast Local Health District (CCLHD) is seeking an experienced and compassionate Clinical Nurse Specialist Grade 2 (CNS2) to take on the role of Clinical Coordinator in the Transitional Aged Care Program (TACP).

About The Opportunity
As the Clinical Coordinator CNS2, you will play a pivotal role in supporting older patients at a critical point in their care journey - post-hospital discharge. You will coordinate timely access to the Transitional Aged Care Program by reviewing referrals, liaising with ward teams, ACAT assessors, and external providers, and ensuring smooth admissions to the program. In this role you will:

About You
We are looking for someone who has:

Current registration with AHPRA
as a Registered Nurse (Division 1), relevant post-registration qualifications, and at least 3 years' experience in their clinical specialty, with a strong background in aged or transitional care nursing.

Demonstrated clinical leadership and coordination skills
, with the ability to effectively manage referrals, communicate across multidisciplinary teams, and mentor colleagues in best practice aged care.

Proven commitment to person-centred care
, upholds professional nursing standards, and actively contributes to the development and improvement of clinical processes and policies.

Excellent clinical judgment and the ability to work proactively
in a dynamic environment, balancing competing priorities while delivering high-quality, compassionate care.

Commitment to ongoing professional development
and strives to enhance patient outcomes through collaborative and innovative approaches to transitional aged care.
