As our Care Coordinator / Care Partner, you'll play a key role in shaping and tailoring care plans that reflect each client's unique goals, preferences, and circumstances. These plans aren't set-and-forget; you'll adapt and refine them as our client's lives change, ensuring support always stays aligned with aged care standards and their well-being.

You'll walk alongside clients on their care journey—many of whom have faced homelessness, mental health challenges, or complex life experiences. With a trauma-informed approach, you'll listen deeply, advocate respectfully, and foster an environment where clients feel safe, supported, and empowered to make choices about their care. Your ability to build trust, form respectful connections, and communicate with warmth will make a lasting difference—not only in the quality of care delivered, but in the confidence and dignity of those you support.

This role could also be perfect for someone who is sitting at Coordinator level, looking to develop further in the aged care space.

A day in the role could see you:

  • Coordinating care for clients with needs ranging from moderate to high and complex, making sure they receive the right support at the right time.
  • Carrying out thorough assessments to understand each person's circumstances, goals, and preferences.
  • Developing, reviewing, and adjusting person-centred care plans in line with both client needs and regulatory requirements.
  • Overseeing the smooth delivery of services under the Support at Home framework within individual budgets
  • Maintaining clear, accurate, and confidential records in line with privacy and documentation standards.

You'll stand out if you have:

  • A Certificate IV in Aged Care or you may even be an Enrolled Nurse
  • Experience in a coordinator level role (great if you do, but not essential), with bonus points for mental health or aged care experience
  • Strong communication skills, a collaborative approach, and the ability to resolve client issues with care and professionalism.
  • Experience supporting vulnerable older people using a person centred, trauma informed approach
  • Strong computer skills and an eagle eye for detail in documentation and record keeping.

About Us

For 60 years, St Bart's has supported people experiencing homelessness and severe and persistent mental health issues as they rebuild their lives. We started as a small homeless shelter for men and have grown into a progressive and inclusive organisation providing support to hundreds of Western Australian men, women and families across four key service areas. Our goal is to provide safe, supported accommodation and services to people who need it the most, regardless of background, age, gender or ethnicity.
