The Role

As an NDIS Support Coordinator, you'll work closely with participants, families, and service providers to implement NDIS plans, build capacity, and navigate complex systems. You'll be the bridge between goals and outcomes — ensuring supports are aligned, sustainable, and culturally safe.

Key Responsibilities

  • Coordinate and monitor supports aligned with participants' NDIS plans
  • Build participant capacity for choice and control
  • Liaise with providers, clinicians, and informal supports
  • Maintain accurate, audit-ready documentation
  • Respond to risk and escalation in line with duty of care and NDIS Code of Conduct
  • Advocate for participant rights and access to services
  • Support transitions, reviews, and plan reassessments

What You'll Bring

  • Experience in NDIS Support Coordination or case management
  • Strong understanding of the NDIS framework, including psychosocial and complex support needs
  • Excellent communication, documentation, and boundary-setting skills
  • Ability to work independently and collaboratively across sites
  • Commitment to participant empowerment, inclusion, and ethical practice
  • Relevant qualifications in Social Work, Community Services, or equivalent
  • Current Police Check, WWCC, NDIS screening check, First Aid and CPR and driver's licence required

Job Type: Full-time

Pay: $75,000.00 – $90,000.00 per year
