About the role

We are seeking a dedicated and experienced Support Coordinator to join our team in Melbourne CBD for a permanent full-time role. Initial training will be conducted for the first week. The role is remotely based, with mandatory in office days roughly three days per month. You may be required to attend the office for additional days to support your learning, development and support team culture.

As a Support Coordinator you'll empower individuals with disabilities to navigate the NDIS and connect with the right services to achieve their goals. You'll work closely with clients, families, and service providers to coordinate supports that truly make a difference.

Responsibilities include:

· Support clients and families in accessing and coordinating services.

· Build relationships with internal and external stakeholders.

· Implement supports outlined in NDIS plans.

· Help clients develop skills to coordinate support and engage in the community.

· Empower clients to understand and implement their plans.

· Be responsive to client needs, especially in crises.

· Conduct regular progress reviews and report to the NDIS.

Skills and Experience:

· Understanding or knowledge of the NDIS

· Prior experience in Support Coordination and/or a similar role

· Familiarity with psychosocial disabilities (advantage)

· Strong communication skills

· Demonstrable problem-solving skills

Requirements

• Valid driver's licence
• NDIS Worker Screening Check
• Working with Children's Check (WWCC)
• NDIS Orientation Module: "Quality, Safety & You"
• Relevant qualification in disability services, social work or similar (desirable)
• Australian working rights (with no restrictions)
