Key responsibilities

  • Plan interpretation and implementation: Help participants understand their NDIS plan, including their funding, goals, and the services they can access.
  • Provider connection and coordination: Find and connect participants with appropriate service providers for community participation, living arrangements, and other needs.
  • Support coordination: Help participants plan and coordinate a mix of supports to work together effectively, including mainstream, community, and disability-specific options.
  • Budget management: Monitor and ensure participants are using their plan budgets effectively and provide support in managing their funding.
  • Capacity building: Develop the participant's skills and confidence to manage their own supports and make informed decisions, preparing them for future plan reviews.
  • Barrier identification: Work with participants to identify and address any barriers that might prevent them from accessing the support they need.
  • Relationship building: Liaise with family members, other service providers, and community organizations to provide holistic support.
  • Reporting and review preparation: Prepare reports for the NDIS to showcase progress and assist participants in preparing for NDIS plan reviews.
  • Crisis management: Help participants prepare for unexpected events by developing crisis plans and connecting them with relevant services if needed.

Essentials:

  • Needs to have previous experience in NDIS support coordination
  • References for previous job experience.
  • Minimum 4 days required for position
  • Drivers licence required
  • NDIS screen check
  • Certificate in industry required or studying
  • NDIS Worker Orientation to be completed or willing to do so.

Job Types: Full-time, Part-time, Fixed term

Contract length: 6 months

Pay: $61,719.30 – $80,000.00 per year

Expected hours: No less than 25 per week

Education:

  • Certificate I - IV (Preferred)

Language:

  • English (Required)

Licence/Certification:

  • Must be studying or have a cert III Disability support (Required)

Willingness to travel:

  • 25% (Required)
