NDIS Specialist Support Coordinator

About the role

We're seeking experienced NDIS Specialist Support Coordinators and Support Coordinators to join our dynamic team at Approved by Frankie. We work closely with participants, families and other agencies to get the most favorable outcomes.

What you'll be doing

• Develop individualised support plans for NDIS participants, ensuring their unique needs and preferences are met
• Coordinate and facilitate the delivery of supports and services from a range of providers, monitoring progress and adjusting plans as needed
• Build strong relationships with clients, their families and other agencies involved to understand their needs and preferences
• Advocate on behalf of clients to ensure they are receiving the appropriate level of NDIS funding and supports
• Collaborate with a network of service providers, government agencies and community organisations to coordinate care
• Maintain detailed case notes and records to comply with NDIS and organisational requirements
• Provide guidance and mentoring to more junior members of the support coordination team and feedback to ensure continuous improvement

What we're looking for

• Minimum 3 years' experience in a similar NDIS support coordination or case management role
• Demonstrated understanding of the NDIS and the ability to navigate the scheme's processes and requirements
• We would look very favourably on applications from people who have a very good working knowledge of hospital discharge processes and protocols and/or have a good working understanding of the interface between the Justice system and NDIS
• Excellent communication and interpersonal skills, with the ability to build trust and rapport with clients
• Strong problem-solving and decision-making skills, with a focus on achieving positive outcomes for clients
• Familiarity with the disability sector and the range of supports and services available
• Relevant tertiary qualifications in Allied health, Social Work or community services field
• Highly organised with strong attention to detail and record-keeping
• Willingness to travel
