We are currently seeking an experienced, passionate, and skilled Support Coordinator to join our dynamic team. Our head office is based in WA. You will be able to work fully remote from Sydney and the Greater areas.

In this role, you'll:

  • Support NDIS participants with complex needs to build capacity and make informed choices about their plan implementation.
  • Collaborate with our NDIS service coordinators, allied health professionals, and key stakeholders to design and maintain tailored supports.

Key Responsibilities

As a Support Coordinator, your responsibilities will include, but are not limited to:

  • Building and maintaining strong, collaborative relationships with participants, families, service providers, and stakeholders.
  • Supporting participants across the six key domains of NDIS Support Coordination:

  • Communication – ensuring accessible and effective communication strategies are in place, tailored to each individual's needs and preferences.

  • Social Interaction – encouraging and facilitating social engagement and community participation.
  • Learning – supporting access to education, skill development, and lifelong learning opportunities.
  • Mobility – coordinating transport and mobility supports to enhance independence and access to services.
  • Self-Care – ensuring support is in place for personal care, hygiene, health, and wellbeing.
  • Self-Management – empowering participants to understand and manage their own NDIS plans, goals, and support networks.
  • Assisting participants to build their capacity to make informed choices and coordinate their own supports where possible.
  • Regularly reviewing and updating participant documentation to ensure alignment with not only their evolving goals and needs; but also with the Practice Alerts issued by the NDIS.
  • Overseeing the implementation and monitoring of NDIS plans with a person-centred, strengths-based approach.
  • Promoting and embedding continuous quality improvement across all service delivery areas.
  • Attending events such as expos, networking events, training sessions to build a strong portfolio and assisting the organisation in developing a strong portfolio of participants.

What We're Looking For

  • Minimum 2 years' experience as a Support Coordinator.
  • Deep understanding of NDIS Practice Standards, restrictive practices, and behaviour support.
  • Experience with complex case management, and service coordination.
  • Strong leadership, interpersonal, and communication skills.
  • Proficient in managing compliance, documentation, incident reporting, and care planning.
  • Confident in using PRODA and other platforms such as BrevityCare, MYP, ShiftCare and others and high-level admin capabilities.

Essential Requirements

  • Certificate IV in Community Services (or working towards).
  • Current Working with Children Check (WWCC).
  • NDIS Worker Screening Check and valid Driver's Licence (State-based)
  • Completion of the following:

  • NDIS Worker Orientation Module – Quality, Safety and You

  • Supporting Effective Communication Module
  • New Worker NDIS Induction

On Successful Appointment

You will be supported to complete:

  • Trauma-Informed Practice & Incident Management Training
  • Medication & Infection Control Training
  • Mandatory Reporting (Child Sexual Abuse)
  • Cultural Awareness & Professional Boundaries Training
