About the Role:

We are seeking a motivated and skilled NDIS Support Coordinator to provide Coordination of Supports for participants with relevant NDIS funding in their plans. This role involves direct participant support, service planning, and coordination with providers while applying SRV (Social Role Valorisation) principles to promote meaningful outcomes for participants.

Key Responsibilities:

  • Provide Coordination of Supports for participants, assisting with goal setting, support planning, and community participation.
  • Develop COS service agreements and negotiate service agreements with providers in consultation with participants, families, and carers.
  • Network with service providers to facilitate choice and control for participants.
  • Monitor participant funds via the NDIS provider portal to ensure optimal achievement of participant goals.
  • Assist participants through Supported Independent Living (SIL) processes.
  • Liaise with service providers to obtain progress reports and prepare for scheduled plan reviews.
  • Develop Support Coordination End of Plan Reports and attend plan review meetings with the NDIS where required.
  • Assist participants, families, and carers to navigate NDIS systems, including myGov and participant portals.
  • Ensure confidentiality of all GCCL information.

We'd Love to Hear from You If You Have:

  • Experience in Coordination of Supports, Service Delivery, or related disability services roles.
  • Strong interpersonal, communication, and conflict resolution skills.
  • Knowledge of the NDIS, participant plans, and service coordination.
  • Ability to manage a caseload of participants effectively.
  • Lived experience of disability (personally or as a carer) is highly regarded.
  • Proficiency in IT systems including MS Office and NDIS portals.
