Job Description:

The AOD (Alcohol and Other Drugs) Program Facilitator works within a multidisciplinary team to address substance use issues among individuals in custody. The role involves conducting assessments, facilitating harm reduction programs, and developing treatment plans aimed at minimizing risks and promoting recovery. The facilitator works closely with both internal and external stakeholders to ensure continuity of care and support for individuals upon release. This position emphasizes trauma-informed care and harm minimization, aligning with national health standards to ensure quality service delivery.

Key Duties:

  • Conduct assessments and screenings related to substance use, mental health, and risk factors.
  • Facilitate harm reduction and release preparation programs, such as Prison-Related Harm Reduction (PRHR) and Naloxone education.
  • Develop individualized treatment and counseling plans, with a focus on trauma-informed care and relapse prevention.
  • Collaborate with health services and reintegration programs to ensure ongoing support post-release.
  • Provide training and supervision to staff and peer educators on harm reduction techniques.
  • Accurately document case notes and manage data for reporting and evaluation purposes.

Skills and Experience:

  • Relevant qualification in social work, counseling, psychology, nursing, or AOD-specific certifications.
  • Experience in AOD assessment, harm minimization, and recovery support for individuals with substance use issues.
  • Knowledge of harm reduction strategies and experience facilitating group programs on drug awareness and relapse prevention.
  • Strong communication skills, with the ability to build relationships across multidisciplinary teams.
  • Proven ability to maintain accurate case notes and meet performance targets.
