Location: Just 35 minutes from Geelong
Employment Type: Part-Time - Permanent 8 hrs per fortnight

About the Role
The Continence portfolio nurse is a key member of the Residential Aged Care nursing team. The position aims to promote optimal continence management for patients and residents of Hesse Rural Health ensuring their dignity and quality of life is maintained.
Key Responsibilities:
Key responsibilities include developing and maintaining continence management care plans, undertaking comprehensive continence assessments and ensuring they are regularly reviewed. In addition, providing continence management education to the care team and leading continuous improvement initiatives in continence management.
