Job Overview

We are seeking a skilled and compassionate Community Health Nurse to join our dynamic team. As a key member of our community health team, you will play a vital role in delivering high-quality patient-centered care to clients in their homes.

Key Responsibilities:
  • Provide nursing care and support to clients, focusing on promoting independence and self-management.
  • Conduct home visits and assessments to identify client needs and develop individualized care plans.
  • Collaborate with multidisciplinary teams to ensure comprehensive care.
  • Administer medications, perform wound care, and provide continence management as required.

To be successful in this role, you will have current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as an Enrolled Nurse (Division 2), a Nursing and Midwifery Board of Australia approved qualification for medication administration, and demonstrated effective clinical communication skills.
