We're Hiring: Support Coordinator (Full Time)

Aus Care Community Services is seeking a passionate and experienced 
Support Coordinator
 to join our growing team in 
Georges Hall, NSW
.

In this role, you'll manage a caseload of 
30+ NDIS participants
, helping them understand, implement, and optimise their NDIS plans. You'll work closely with participants, families, carers, allied health professionals, Justice Liaison Officers, and community services to deliver outcomes that build independence, capacity, and meaningful participation.

What You'll Do:

  • Lead participant intake, engagement, and personalised care planning
  • Break down and optimise NDIS plans to maximise outcomes and funding utilisation
  • Collaborate with allied health teams, community organisations, and justice stakeholders
  • Assist participants with housing and accommodation pathways
  • Maintain accurate documentation and reports in line with NDIS standards

What We're Looking For:

Bachelor's degree in Psychology, Social Work, or related Social Science field

Minimum 
3 years' experience
 in NDIS Support Coordination or similar role

Strong understanding of NDIS framework, behaviour support, and safeguarding

Experience with complex needs and justice system involvement highly regarded

Excellent organisational, advocacy, and communication skills

Current WWCC, NDIS Worker Screening, Police Check, First Aid/CPR, and Driver's Licence
