The role

Key responsibilities include:

  • face to face client commencement home visits and client reviews
  • support the implementation of client focused goal directed care plans and services
  • managing client intake and referrals
  • administrative tasks, including data entry and maintenance in electronic systems
  • provision of advice and support to clients, carers, families and other stakeholders

Our ideal candidate

To be successful in this role you will have:

  • experience working in a Health or Community Care environment that supports independent living and participation in the community
  • clinical and or allied health qualifications highly desirable
  • ability to use initiative, liaise with stakeholders, work with minimal supervision and work collaboratively within a team environment
  • competent skills in Microsoft Word, Excel, and the ability to adapt to new systems and processes; experience with Carelink+ is desirable
