About the Role

As a Support Coordinator, you will assist NDIS participants to understand and implement their plans, connect with appropriate supports and services, and build the skills needed to navigate the NDIS with confidence and independence.

You will work closely with participants, families, and service providers to ensure services align with each individual's goals and aspirations.


Key Responsibilities
  • Work with participants to understand their NDIS plan and funding.
  • Connect participants with appropriate supports, including mainstream, community, and funded services.
  • Coordinate service delivery and support implementation.
  • Monitor progress and ensure supports are effective and aligned with goals.
  • Build participant capacity to manage and direct their own services.
  • Maintain accurate, up-to-date documentation, case notes, and reporting.
  • Comply with NDIS legislation, reporting, and quality standards.

We're looking for someone who is passionate about making a difference, highly organised, and able to build strong relationships with participants and providers.

Essential Criteria

  • Previous experience in NDIS Support Coordination or related field (e.g., case management, mental health, disability support).
  • Minimum 2 years of NDIS Support Coordination experience is required.
  • Strong understanding of the NDIS framework and participant pathways.
  • Excellent communication and interpersonal skills.
  • Ability to manage a caseload and prioritise tasks independently.
  • High level of accuracy and attention to detail in record keeping and reporting.
  • Relevant tertiary qualification in Community Services, Social Work, Psychology, or related field.
  • Current driver's licence and reliable vehicle.
  • Current NDIS Worker Screening Check, Working With Children Check, and Police Check (or willingness to obtain).
