About the role

This part-time 30+ hours per week, (potentially full time) Support Coordinator role at Ignite Community Services' in Ipswich, QLD is an excellent opportunity to make a real difference in the lives of people with disabilities. As a Support Coordinator, you will be responsible for coordinating and facilitating the delivery of high-quality support services to participants, ensuring they can achieve their goals and live independently.

What you'll be doing

  • Developing and implementing individualised support plans for participants based on their unique needs and goals
  • Coordinating the services of various support providers to ensure seamless delivery of support
  • Monitoring the effectiveness of support plans and making adjustments as necessary
  • Advocating on behalf of participants to ensure they receive the support and services they require
  • Maintaining detailed records and preparing reports to document the progress of participants
  • Collaborating with other members of the Ignite Community Services' team to provide a cohesive support network

What we're looking for

  • Relevant qualifications in a field such as social work, community services or disability studies
  • Demonstrated experience in a similar support coordination or case management role
  • Strong understanding of the NDIS and the support needs of people with disabilities
  • Excellent communication and interpersonal skills, with the ability to build trusting relationships with participants
  • Proven problem-solving and conflict resolution skills
  • Commitment to the principles of person-centered practice and the ability to empower participants
  • Strong organisational and time management skills
