About your new role

You are working as part of a transdisciplinary team which includes Intake and Assessment Workers (including both professionals and Lived Experience), Mental Health Clinical Support Coordinators, a Peer Practice Lead and a Clinical Lead. You will report to the Service Manager for the Darling Downs West Moreton Medicare Mental Health phone service.

As Mental Health Clinical Support Coordinator, you will support the delivery of high-quality mental health intake, assessment and referral services for individuals in the community who experience mental health issues and are seeking support. A priority of the role is to connect people seamlessly to the most appropriate local service to meet their identified needs, consistent with the 'stepped care' approach.

About the Program

The Medicare Mental Health phone service (MMHps formerly Head to Health Phone Service) is an exciting new service connecting the Darling Downs West Moreton communities to the most appropriate services that meet their identified needs. When calling the 1800 number, consumers will be greeted by mental health professionals offering mental health service navigation into and between all community mental health services. This consists of Medicare Mental Health Centres, Headspace, and other community-based mental health services, including those provided by Queensland Health, Non-Government Organisations, and private providers.

The MMHps is part of a national Medicare Mental Health brand, that enables a consistent triage and streamlined transfer or referral to the most appropriate services. A key objective of this service is local service integration to address fragmentation and offer a seamless care pathway for consumers to receive the right level of care at the right time to meet their mental health needs.

This is primarily a phone-based service, taking direct incoming referral calls through an established 1800 mental health intake number. The service uses the Initial Assessment and Referral Decision Support Tool (IAR-DST) alongside other assessments and tools, to support a nationally consistent evidence-informed initial assessment and referral process. The team is made up of staff from diverse professional and personal backgrounds including clinical, non-clinical, and lived experience. Staff worktogether in a transdisciplinary approach to support people to reach the appropriate service, prioritising consumer choice and agency.

The MMHps operates from 8:30am to 5:00pm Monday to Friday. Opening hours may be adjusted over time in response to community needs and service demands.

What you will bring to the role

  • Proven experience at working within multidisciplinary teams within the area of mental health service delivery.
  • Demonstrated skill in the application of recovery-based approaches in a clinical setting.
  • Experience working as a member of a multi-disciplinary (or trans-disciplinary) team with the ability to inspire and facilitate teams toward shared objectives and creating environment that encourages innovation and excellence.
  • Demonstrated experience in preparation of reports with recommendations.
  • Demonstrated ability to form and nurture partnerships with government and non-government mental health, emergency, and community service providers.
  • Formal tertiary qualifications in Nursing, Psychology, Social Work, Occupational Therapy or related
  • Current full registration as a Registered Nurse with current annual practicing certificate from the Australian Health Practitioner Regulation Agency (AHPRA), OR
  • Current full registration as a Psychologist, Occupational Therapist or Registered Nurse with current annual practicing certificate from the Australian Health Practitioner Regulation Agency (AHPRA), OR
  • Current full registration as a Social Worker with the Australian Association of Social Workers (AASW)

  • Current Working with Children Check - required before commencement of work
  • Australian working rights
  • A valid driver's licence
  • Appropriate evidence of vaccinations as per the Vaccine Preventable Diseases (VPD) requirements: Hepatitis B, Measles, Mumps. Rubella (MMR), Varicella (Chickenpox) and Pertussis (Whooping Cough)
