About the role:
This dual role of predominantly rostering with client care coordination requires a detail orientated person with excellent communication skills.
- Rostering all client hours within the company
- Coordinating staff availability
- Coordinating and overseeing the NDIS planning and coordination of support to participants
- Coordinating the intake process including the initial meet and greet as well as negotiating Service Agreements with participants and support coordinators
- Assisting and maintaining the Roster of Care outlining the participants' support needs
- Supervising Support Workers in the provision of quality person-centered services
- Being actively involved with the development, implementation and review of support plans required for participants
- Coordinate the delivery of daily living supports enabling participants to achieve their goals through the implementation of their NDIS plans
- Develop and maintain professional relationships with a diverse range of internal and external stakeholders, demonstrating effective participant support and service delivery
- Coordinate team processes, including client reviews, team meetings and program reviews
Qualification/Requirements:
- Diploma in Business or relevant industry experience
- Cert 4 in individual support or relevant industry experience
Skills:
- Strong interpersonal communication skills (Written and Oral),
- Ability to think on your feet, Work calmly under pressure
- Strong time management, negotiation, problem solving and organisational skills
- Ability to work with people with multiple and complex disabilities and behaviour support requirements and provide Person-centered care
- Capacity to work effectively in leadership positions
