About the role
A fantastic opportunity has become available for a Clinical Care Partner to join our Support at Home (SaH) team.
You will be an experienced health professional with a strong clinical background, demonstrated interdisciplinary leadership capability, and a sound understanding of SaH services. You will be accountable for delivering and managing client care management activities, providing ongoing planning, monitoring, service coordination, education and direct nursing tasks with a strong focus on wellness, reablement, and culturally safe care.
The Specialist Clinical Care Partner will drive team performance, achieving KPI's, ensuring financial sustainability and that services address both clinical and non-clinical needs.
You will provide guidance to a team of care partners and support clients though collaborative goal orientated care planning, to promote independence wellbeing and meaningful engagement in everyday life.
The role combines advanced clinical judgment with case management.
Key Responsibilities
· Conduct initial and ongoing assessments to understand physical, emotional, and social needs, identifying risks and tailoring care plans with clients and families.
o Identification of Clients needs and goals
o Development of client directed goal orientated care plans
o Regular monitoring and review of services to ensure goals are met
· Develop, review, and update person-centred care plans aligned with client goals and Support at Home funding.
· Support clients to understand their funding, choices, and rights, and advocate for access to appropriate services.
· Ensure services are aligned with National Safety and Quality Health Service standards, the National Standards, and the Aged Care Quality Standards, and ensure appropriate actions are effectively implemented to address areas of under-performance.
· Monitor and periodically audit all Care plans, Budgets, SaH scheduling and quality of service delivery by Associated Providers and Sole Traders
· Monitor spending to prevent underspends and overspends and align resource allocation with clients changing needs, including temporary increases during illness or recovery
· Ensure all team members clearly communicate to clients when items are claimable, partly claimable or un-claimable, ahead of the expense being incurred and invoices submitted for claiming to Services Australia
· Working closely with Finance, Scheduling and other relevant staff, to ensure the most effective and efficient allocation of staff hours and financial resources, at all time
· Maintain accurate, timely care notes and ensure documentation supports outcomes under the Strengthened Aged Care Quality Standards, particularly Standard 3 (Care and Services) and Outcome 5.1 (Clinical Governance).
· Conduct clinical assessments for clients using standardised tools, within program specific requirements.
· Provide other specific services (including direct care) for clients within your scope of practice as a clinical health professional
Qualifications and Experience
· Nurse or Allied Health professional holding a current AHPRA registration and unlimited work entitlements
· Possession of a valid Motor Vehicle Licence.
· Access to your own registered, reliable, and comprehensively insured motor vehicle.
· Willingness to undergo a National Criminal Check (not older than 3 years)
· Demonstrated understanding and sound knowledge of client centred, community-based care services delivery
· Excellent verbal, written and communication skills
· Demonstrated knowledge of contemporary technology and practices in a community/health/disability or aged care setting
· A working knowledge, interpreting workplace agreements/awards, in a scheduling/rostering context
· Demonstrated experience working as part of a team and taking a leading role
· Previous experience working to KPI's and tight deadlines,
· Experience delivering services, maximising consumer benefit within budget constraints;
· A good understanding of Commonwealth funding in a community/health/disability or aged care setting
