Albany Community Hospice is a well respected, not-for-profit organisation providing specialised palliative care. With strong community support, we are committed to ensuring every individual experiences end-of-life care with choice, dignity, compassion, and understanding.

We are currently seeking a Support Coordinator who will play a vital role in planning, coordinating and facilitating person-centred, evidence-based support for patients, families, and carers at one of the most meaningful times in their lives.

As our Support Coordinator, you will:

  • Build trust with patients and their loved ones, supporting person-centred decision-making.
  • Develop and implement tailored care and support plans that meet individual emotional, psychological, practical, and spiritual needs.
  • Provide grief support for both pre-bereaved and bereaved, identifying those at risk of complex grief.
  • Assist with care coordination, including access to aged care, respite, or supported accommodation where needed.
  • Offer advance care planning support and advocate for patients and families to ensure care aligns with their values and choices.
  • Source and share up-to-date palliative care resources to empower patients and carers.

To be successful in this role, you will bring:

  • Qualifications and/or experience in social work, counselling, community services, or a related field.
  • A deep commitment to person-centred, compassionate care.
  • Strong communication, interpersonal and advocacy skills.
  • Experience in grief, loss, or palliative care support (highly desirable).
  • An ability to work collaboratively within a multidisciplinary team.
  • Capacity to coordinate resources, assess risk, and adapt to complex emotional environments with sensitivity.
  • A current driver's licence and relevant clearances.
