About the role
As a Jr. Service Coordinator, you will coordinate participant onboarding, align supports to individual goals, manage rosters, and maintain strong communication with participants, families, support workers, and external stakeholders. You will help ensure services meet NDIS Quality and Safeguards Commission standards while promoting safety, inclusion, and continuous improvement.
Key responsibilities
- Client onboarding and intake
- Act as first contact for new participants and families
- Complete intake assessments and support smooth transitions into services
- Care and support coordination
- Develop and implement individualised support plans
- Track goals and wellbeing and adjust supports as needs change
- Provide guidance and problem solving for participant or family concerns
- Staffing and rostering
- Build and maintain reliable rosters
- Coordinate shift coverage and manage changes
- Assist with recruitment tasks including interviewing and onboarding support workers
- Communication and documentation
- Maintain accurate records including service delivery and incident reports
- Provide updates and reports to management as required
- Quality, safety, and compliance
- Ensure services meet NDIS Quality and Safeguards Commission standards and organisational policies
- Identify and escalate risks or incidents promptly
- Advocacy and empowerment
- Champion participant rights, choice, control, and social inclusion
- Professional development
- Engage in ongoing learning and apply best practice to improve outcomes
What you will bring
Required
- Experience in service coordination or client services in disability, aged care, or community services
- Strong administrative and organisational skills with experience in rostering and workforce coordination
- First Aid and CPR certification
- NDIS Worker Screening Check and Working with Children Check
- National Police Check
- Evidence of right to work in Australia
- COVID 19 vaccination certification
- Current driver license and access to a reliable, insured vehicle
Desirable
- Certificate IV or higher in Disability, Community Services, Case Management, or related field
- Experience working with NDIS participants, families, and support workers
- Knowledge of the NDIS framework, service agreements, and compliance
- Familiarity with disability legislation, safeguarding, and person centred practice
- Experience supporting recruitment, onboarding, and performance management
