Service Delivery Coordinator – Southern Star Care

Location: Mulgrave, VIC

Job Type: Full-Time

Salary: $70,000-$80,000 based on experience

The Role:

We are seeking a proactive and organised Service Delivery Coordinator to join our team. This role will be pivotal in building rapport with participants to understand their situation, unique needs and goals, and coordinate service delivery ensuring Support Workers are working with participants to meet outcomes of each individual plan.

Key Responsibilities:

  • Work with participant, family/carers, relevant support coordinators and other professionals to support the participant's individual goals
  • In conjunction with rostering team, ensure rosters are in place and shifts appropriately covered for any planned and unplanned absences and ensuring assigned support workers are the 'right fit' for participants.
  • Review and ensure compliance standards amongst support workers through detailed and sufficient shift notes and completion of incident reports.
  • Ensure staff and client profiles are up to date, to create a smooth transition of services.
  • Monitor the safety and well-being of the participants and report/escalate any concerns.
  • Collaborate with participants, families, staff and other stakeholders to understand and resolve concerns, escalating to the Management as required

About You:

  • Previous experience as a Service Delivery Coordination in the NDIS sector
  • Excellent communication and interpersonal skills
  • Strong organisational and problem-solving abilities
  • Ability to work collaboratively in a team environment
  • Empathy and a genuine passion for supporting people with disabilities
  • Ability to offer suggestions, new ideas and step out of the role and be a leader
