Reports to: Service Delivery Manager / Operations Lead

Travel: Required across Melbourne metro | Work mode: On‑site leadership + remote administration

Salary: SCHADS Level 5

About Care Fast:

Care Fast is a registered NDIS provider, recognised as one of Melbourne's leading specialists in safe hospital discharge and high-intensity SIL. We run person-centred homes where participants build skills, independence and community connection.

Role Purpose:

This is a player‑coach position. You will lead a SIL home, mentor a small team of Disability Support Workers, and work selected support shifts to model best practice. You'll oversee day‑to‑day operations, uphold quality and safety standards, and ensure supports align with each participant's NDIS goals and plans.

Key Responsibilities:

· Lead, mentor and supervise Support Workers; build rosters and approve timesheets.

· Work rostered shifts alongside staff to provide on‑the‑job coaching and model best practice.

· Oversee daily home operations: household budget, property upkeep, WHS and environmental safety.

· Ensure supports align with participants' NDIS goals, assessments and support plans.

· Implement, monitor and review Behaviour Support Plans; facilitate staff training and refreshers.

· Act as primary point of contact for participants, families/guardians, Support Coordinators, allied health and NDIS stakeholders for service‑delivery matters.

· Maintain documentation and progress notes; complete incident/hazard reporting and meet NDIS Quality & Safeguarding Framework requirements.

· Coordinate appointments, community access and transport logistics; solve day‑to‑day service issues promptly.

· Provide after‑hours on‑call support as rostered.

Skills & Experience Requirement:

· Diploma in Community Services, Leadership & Management, or equivalent experience.

· 2+ years in a leadership role within disability services; SIL/group home experience preferred.

· Working knowledge of the NDIS, Practice Standards, and safeguarding requirements (incl. incident management & restrictive practices).

· Experience in rostering, supervision, coaching and performance management.

· Confidence supporting mental health and implementing Behaviour Support Plans; training/assessor experience valued.

· Excellent communication, de‑escalation and conflict‑resolution skills.

· Sound administration skills; accurate documentation and note‑keeping.

· Basic property coordination and household budget management capability.

· Current NDIS Worker Screening Check, Working with Children Check, First Aid/CPR certificate

· Current Australian driver's licence and reliable, insured vehicle.
