An opportunity exists for a Community Home Care Coordinator/ Leader to support older Australians to live independently at home. 

The role will lead the clinical team and coordination for clients in their homes along with the case managers, RNs, Allied Health and care workers to provide high quality care.  

To be successful you will be:

  • Experience leading a clinical team either within community or other aged care services
  • Proven leadership experience in a similar role
  • Understanding of Consumer Directed Care principles
  • High level of customer service skills
  • The ability to work in a fast paced team environment
  • Effective communication, both verbal and written
