About the Role

The Home Care Partner collaborates with the Care Manager - Home and Community Support to understand and address the care needs of existing and prospective consumers. With a primary objective to provide effective and high-quality care coordination to Support at Home programs and consumers. Adopting a person-centered approach to assessment and care planning, the Home Care Partner assists consumers in selecting the services essential for maintaining their independence when developing person-centered service plans and individualised budgets, along with ongoing monitoring, reviewing, and evaluation of the services provided.

Key responsibilities include:

  • Conduct initial assessments for new consumers joining the LHG Home and Community Support
  • Provide a highly responsive, flexible service, based on the principles of 'Individualised Service Planning'
  • Undertake on-call duties as required
  • Actively develop professional knowledge in the field and enhance expertise that ultimately result in better outcomes for customers.
  • Complete a formal review of the care/support plan, involving all key stakeholders, as directed by program guidelines or as change is indicated.
  • Assist with the delivery of care, as required, in scope with qualifications, skills and experience such as transport, social support or personal care
  • Ensure services are client focused and based on dignity, respect, cultural sensitivity and informed choice.
About You
  • Certificate III in Community Services or equivalent work experience – Essential
  • Tertiary qualifications in Nursing, Aged Care or a related field or willingness to work towards – Preferred
  • Previous experience working in a coordination capacity for a Home and Community aged care organisation will be highly regarded.
  • A current Australian driver's licence
  • The ability to work autonomously, self-motivate and operate with minimal supervision is a necessity for success in the role.
  • A strong theoretical approach to working with ageing population and demonstrated practical application of this approach.

Candidates may be subject to a pre-employment medical assessment with the successful candidate requiring current NDIS worker clearance. In accordance with current health guidelines for the aged care sector, candidates are encouraged to have all recommended vaccinations.
