Top Care Community Services Pty Ltd (TCCS) is a registered National Disability Insurance Scheme (NDIS) service provider based in the Northern Territory, primarily providing Support Coordination services. We support our participants to develop sustainable capacity and lead a healthy, content life with enhanced social and family relationships.

We are seeking a compassionate, experienced and well-organised person who has an understanding of NDIS processes to join our team.

As a Support Coordinator, your primary role will be to assist NDIS participants understand and implement their NDIS Plan, connect them with appropriate supports and assist with budgeting their funding. Strong Communication and organisational skills are key to success in this role. The role is hybrid, but the successful candidate is required to undertake visitations with participants in Greater Darwin Region.

Location: Darwin

Key responsibilities:

  • Assist NDIS participants in implementing their NDIS plans, goals and objectives while promoting choice and control.
  • Identify, connect, and coordinate appropriate service providers and community resources that meet the participant's needs in accordance with their allocated NDIS funds.
  • Help participants to monitor their NDIS budgets, ensuring services are delivered within their plan allocations.
  • Empower participants to build their own capacity to manage their support and navigate the NDIS system independently over time.
  • Liaise with various stakeholders including service providers, allied health professionals, and community organisations to ensure seamless service delivery.
  • Provide assistance during times of crisis, ensuring continuity of support and appropriate intervention where necessary.
  • Maintain accurate records of participant progress, develop reports, and communicate regularly with the NDIS regarding plan outcomes and reviews.
  • Ensure compliance with all relevant legislation, NDIS guidelines and maintain up-to-date knowledge of any changes to the NDIS framework.

Selection Criteria:

  • A tertiary qualification in Social Work, Community Services, Mental Health, Disability or a related Allied Health discipline.
  • Strong cultural awareness and experience supporting multi-cultural, Aboriginal and Torres and Strait Islander participants.
  • Must have a minimum of two (2) years' experience as an NDIS Support Coordinator.
  • Demonstrated a solid knowledge and understanding of the NDIS including mandatory reporting, Quality and Safeguards, NDIS Price Guide and relevant legislation's.
  • Experience within community connecting and building relationships with service providers, individuals and/or families/carers
  • Must be able to relay information to participants so they understand.
  • Strong written, administration and computer skills for the creation of case notes, reports, correspondence and other documents in a cloud-based management system.
  • Strong interpersonal skills with a positive attitude and a flexible and adaptive nature to respond to the changing needs of our participants.
  • Excellent organisational and time management skills.
  • Applicants will be subject to a NDIS Worker Screening Check, Working with Children Check, and four (4) NDIS mandatory eLearning modules.
  • National Police Check

  • Australian Drivers Licence, and a reliable vehicle with registration and insurance
  • Right to work in Australia.
