At South Eastern Melbourne Primary Health Network (SEMPHN), we're a not-for-profit working to create a healthier future. From GPs and hospitals to aged care and pharmacies - we connect the dots so people get the right care, in the right place, at the right time.

We support over 1.6 million people across our region, with a focus on aged care, mental health, Aboriginal and Torres Strait Islander health, digital health, and workforce development.

What You'll Do

You'll lead the charge in improving access to treatment and making a real difference for people in our community. Your day-to-day might look like:

  • Build Strong Connections:
    Be the trusted link between GPs, pharmacists, Alcohol and Drug services, and the wider health network.
  • Empower Providers:
    Coach, mentor, and upskill health professionals so they can deliver safe, high-quality care.
  • Make Things Happen:
    Plan and roll out impactful projects that boost access and quality in pharmacotherapy services.
  • Turn Insights into Action:
    Map service gaps, streamline referral pathways, and link providers with the right supports.
  • Bring People Together:
    Lead Peer Learning groups, Advisory Committees, and training that actually make a difference.
  • Champion Change:
    Help embed best practices and drive continuous improvement across the region.

About You

  • You get primary care, AOD services, and what great clinical governance looks like.
  • Bonus points if you have pharmacotherapy or AOD experience
  • Have a valid driver's licence and don't mind hitting the road to connect.
  • You're a people person, easily connecting with everyone you meet building trust, and influencing positive change.
  • You're a great communicator Whether it's presenting, facilitating, or writing, you make complex things clear.
  • You can juggle projects like a pro, keeping on top of timelines, while delivering high-quality outcomes.
  • You're solutions driven, quick on your feet and always ready to solve problems a better way.
