Your Role
We are looking for a Care Partner to join our aged care team on a full-time contract until the end of November 2026, with the possibility of extension. Day to day you will be coordinating aged care clients on Support at Home packages enabling them to live in their own home independently, safely and with dignity. This will involve visiting aged care clients in their home, developing their care plans, coordinating their care, and managing their individualized budget. With the increasing need for clinical support, we are seeking an experienced health professional, with RN qualifications and experience to join out team. Using your experience, you will care manage more complex aged care clients and support our non-clinical Care Partners.
This is a full-time roleand reports directly to the Team Leader, Support at Home. Our offices are based in Chatswood, and this role will also have a community-based component, which requires you to have a drivers' license. There is opportunity for flexible work arrangements including the potential to work from home.
Your Responsibilities:
· Conduct assessments and care planning with a focus on wellness and reablement
· Monitor clinical risks and supports to ensure client safety and wellbeing
· Manage individual care budgets in line with eligibility guidelines
· Maintain accurate case notes and documentation on client supports and situations
· Follow up referrals and care plans, conduct reviews, and assess customer satisfaction
· Build strong relationships with clients, families, and healthcare providers
· Collaborate with external service providers and participate in partnership projects
· Promote Your Side's services through networking, forums, and community events
· Promote Your Side at community events and networking forums
Your Skills and Experience:
· AHPRA registered or Tertiary qualifications in Nursing (Registered Nurse) or a relevant Allied Health profession (e.g. Occupational Therapy, Physiotherapy, etc.)
· Highly developed assessment and case management skills
· Experience in community-based aged care services, particularly Home Care Packages or Support at Home
· High-level customer service and communication skills with the ability to build rapport quickly
· Strong problem-solving and dispute resolution skills
· Confident in describing Your Side's services and identifying client needs and opportunities
· Strong administration and IT skills with attention to detail
· Comfortable working both independently and as part of a collaborative team
· Current driver's licence and access to a registered vehicle
