What is the purpose of this role?
This role will engage people affected by MS and other neurological conditions and provide them with information and referral. The Connect Coordinator will:
- Empower MS Plus clients with information, support and referral to appropriate services.
- Empower MS Plus clients to increase their independence.
- Empower MS Plus clients to increase social and economic participation and build their capacity to actively take part in the community.
Your responsibilities
As a Connect Coordinator some of your key responsibilities would include:
- Responding to contact from internal and external stakeholders. Contact may be via phone, email, webform, webchat or in person.
- Establishing an understanding of a client's needs through person-centred assessment to determine most appropriate intervention and referral pathway.
- Providing support to enable people affected by multiple sclerosis and other neurological conditions to adjust to life changes, demonstrating active listening and empathy in all communications.
- Providing up to date information about multiple sclerosis.
- Presenting information and options to clients empowering them to make informed choices.
- Developing strong rapport with people affected by multiple sclerosis.
- Developing strong and collaborative working relationships with relevant agencies to promote the open sharing of information conducive to positive outcomes.

Skills and experience:
Essential
- Experience working in a health care or disability setting.
- High levels of computer literacy in using a range of MS Office computer packages & other programs.
- Exceptional customer service skills, with the ability to develop effective working relationships.
- Proficient interpersonal, oral and written communication skills.
- Strong organisational, planning and time management skills.
Desirable:
- A Diploma or higher in a related field – Allied Health, Social Sciences, Health Sciences, Disability Services OR relevant experience in the above fields.
- An understanding of multiple sclerosis and other neurological conditions, and the impact they may have on an individual
Pre-employment checks:
If successful, you will need to complete a National Police Check (and possibly an International Police Check).
