ABOUT THE ROLE
As the CHSP Service Coordinator, you will ensure the effective management of CHSP services to Amana Living's Home Care clients, their families, and carers. This role is offered on a full time fixed-term contract ending on the 18th of September 2026. It is working Monday - Friday
Key responsibilities:
- Ensuring coordination of services and level of client contact complies with CHSP & Amana Living guidelines, and where needed contact appropriate referrals to provide further support services.
- Acting as a first point of contact for all CHSP referrals within the designated geographical area from My Aged Care to Amana Living clients and their families.
- Supporting clients and their families to exercise informed choice about the care and services they receive.
- Providing safe and effective services, and support for daily living that meets the emotional, spiritual, and psychological wellbeing needs of clients.
- Coordinating effective personal and clinical care that is best practice, tailored to individual needs and optimises health and wellbeing.
- Contacting clients and conducting Health & Safety and environmental checks at the initial referral, and then annually.
- Providing telephone and face to face support of clients and carers within allocated geographical areas.
- Ensuring Community Support Workers work to the My Aged Care developed care plan.
- Maintaining and updating in a timely manner all client case notes and client files.
- Proactively working towards achieving and maintaining KPI's as set out for the designated geographical region.
- Responding to client complaints and concerns in a proactive and professional manner, investigating and resolving issues in a timely manner.
- Providing after hours on-call duties in accordance with the rostered schedule.
- Identifying, initiating, and coordinating quality improvement activities in relation to client care including quality audits to ensure compliance.
ABOUT YOU
In this role you will build strong, positive relationships with internal and external stakeholders, and apply your superior organisational, analytical, problem solving and time management skills across client relations and service coordination.
Essential Criteria:
- Previous Aged/Home Care experience desirable
- Tertiary qualifications of Certificate IV in Aged Care and/or Community Care.
- Previous experience as a Service Coordinator (or similar).
- Demonstrated ability to work well under pressure, multitask and meet deadlines, with a passion for delivering against targets.
- Exceptional customer service skills and interpersonal communication skills.
- Advanced analytical, organisational and time management skills.
- Proficient with MS Office programs and Client Management databases.
- Valid Police Clearance less than 6 months old (or provide a receipt at interview stage).
- Current C Class Driver's Licence and own vehicle.
- Proof of Covid-19 and Flu 2023 vaccinations
