We are now seeking an experienced and motivated Specialist Service Coordinator to join our passionate team. This role is ideal for someone with a strong background in disability or aged care services, who thrives in a fast-paced environment and is passionate about achieving meaningful outcomes for participants with complex needs.

About the Role

As a Specialist Service Coordinator, you will play a pivotal role in coordinating and overseeing services for participants with high and complex needs. This includes managing staff rosters, ensuring compliance, liaising with families and external stakeholders, and supporting participants to achieve their NDIS goals. You will also provide leadership and guidance to staff, fostering a culture of professionalism, accountability, and care, whilst ensuring compliance with funding and documentation requirements.

Key Responsibilities:

Coordinate and oversee service delivery for Supported Independent Living (SIL) and Community Access participants.

  • Develop and maintain staff rosters, ensuring appropriate coverage with minimal overtime.
  • Provide leadership, mentoring, and performance management to staff.
  • Build and maintain strong relationships with participants, families, and key stakeholders.
  • Conduct regular face-to-face stakeholder meetings to ensure services are aligned with participant goals.
  • Support participants with complex behaviours and ensure appropriate strategies and supports are in place.
  • Ensure compliance with NDIS Practice Standards, funding requirements, and company policies.
  • Work collaboratively with allied health professionals, support coordinators, and external agencies.

About You

You are a skilled and compassionate professional with a proven track record in supporting people with complex needs.Your experience spans across community services/mental health/disability supports, giving you the knowledge and resilience to navigate challenging situations with confidence. You are proactive, solutions-focused, and thrive on building strong relationships with participants, families, and stakeholders.

Essential:

  • Proven experience in a Service Coordinator or Specialist Service Coordinator role within the NDIS, Disability, Mental Health, or Community Services sectors.
  • Strong understanding of SIL and Community Access service models.
  • Demonstrated experience supporting participants with complex behaviours, trauma backgrounds, or mental health conditions.
  • Knowledge of best practice strategies for behaviour support and crisis management.
  • Strong leadership and people management skills with experience mentoring and developing staff.
  • Excellent organisational, time management, and problem-solving abilities.
  • High-level communication and interpersonal skills.
  • Current Driver's Licence and reliable vehicle.
  • Current Blue Card and NDIS Worker Screening Check (or willingness to obtain).
  • First Aid and CPR certification.
