We're partnering with a trusted Not-for-Profit organisation that's dedicated to supporting older Australians to live independently and with dignity. This is a great opportunity to join a passionate and values-driven team in Hawthorn, where people and purpose truly come first.
This full-time, office-based role (Monday to Friday, 9:00 AM – 5:00 PM) offers a supportive, collaborative environment with leaders who genuinely care and back their team.
About the Role
As a Care Coordinator, you'll be responsible for coordinating services and supports for clients receiving Home Care Packages. You'll build meaningful relationships with clients, their families, and service providers to ensure that care plans are person-centred, effective, and compliant.
You'll be the key contact for your client portfolio — managing budgets, scheduling services, and helping clients understand their package spend and available choices.
Key Responsibilities
- Coordinate and oversee Home Care Package services for a portfolio of clients
- Conduct client intake, assessments, and regular reviews
- Develop and manage care plans and budgets aligned with client goals
- Maintain accurate case notes and effective communication with stakeholders
- Use the LOOKOUT system for data management and reporting
- Promote and refer clients to Benetas and other community services
- Support continuous improvement and ensure compliance with quality standards
About You
- Experience in Aged Care or Home Care Packages is essential
- Knowledge of Support at Home reforms, CHSP, and VHC programs
- Excellent communication, coordination, and documentation skills
- Compassionate, proactive, and client-focused
