About The Role

$80,000 to $90,000 plus Vehicle allowance plus Super

Reporting to the Regional Manager, located in West Sydney, you will supervise and coordinate teams delivering Supported Independent Living (SIL) services, ensuring high-quality, person-centered supports for individuals with disability in residential care settings. The Team Leader, in addition to their direct care and Client focused duties, will supervise staff, oversee service delivery, and ensure compliance with legislative, policy and quality standards, while upholding the rights, preferences, and wellbeing of Sylvanvale Clients.

The role includes:

  • Conduct Client intake as well as ongoing periodic assessments.
  • Develop, implement, and review individualized support plans in collaboration with Clients and their support networks.
  • Ensure Client choices, rights, cultural identity, and dignity are respected and embedded in daily practice, and within their individualized support plans.
  • Liaise with Client support networks to implement or link the Clients to internal or external activities and programs.
  • Act as the key contact for families, guardians, allied health providers, and other stakeholders.
  • Actively monitor and respond to incidents, concerns or changes in Clients' health and wellbeing.
  • Supervise daily operations of residential care homes to ensure safe, engaging, and quality care.
  • Build and maintain a positive team culture through reflective supervision, coaching and performance management.
  • Oversee referral of Clients to external agencies for additional outsourced support as required.
  • Coordinate team meetings, supervision sessions, and facilitate workforce planning and rostering.
  • Monitor and report on service delivery outcomes, incidents, risks, and compliance matters.
  • Participate in internal audits, implement improvements, and contribute to operational planning.

Criteria and Experience:

  • Minimum Diploma-level qualification in Disability, Community Services or related field.
  • 2+ years in a similar residential care role.
  • Demonstrated experience in a supervisory or leadership role within disability or residential care services.
  • Strong understanding of the NDIS Practice Standards and Quality & Safeguards Framework.
  • Knowledge of active support, positive behavior support, and rights-based care.
  • Strong interpersonal, communication, and stakeholder engagement skills.
  • Experience in staff development, rostering, and performance management.
  • Sound decision-making, conflict resolution, and problem-solving abilities.
  • Familiarity with budgeting, funding utilization and cost centre management.
  • Competency in digital tools such as Microsoft Teams, Office Suite and client management systems.

Desirable:

  • Previous experience in a team leader/supervisory role in a residential care facility.
  • Experience in service development, Client onboarding and change management.
  • Knowledge of WHS legislation, psychosocial risk management, and incident response.
  • Previous experience in a culturally diverse or trauma-informed service setting.
  • Your flexibility to work across multiple residential sites in the Greater Sydney area, if required, along with your availability to work non-standard hours, as needed, is essential.

Successful applicants will require a current NSW Drivers Licence and First Aid certificate. An offer will be subject to a police clearance, a pre-employment medical, 100 points of ID and a NDIS Worker Screening Check.
