About the Role
We are seeking Clinical Care Partners — Enrolled and Registered Nurses — to join us as independent contractors in the Newcastle and Hunter Region. This role offers attractive remuneration, flexibility to work to your own schedule, and incentives for growing your client base. You'll support clients funded under the Support at Home program as well as privately funded pathways, delivering care management and nursing services across diverse client groups. All the while, you'll make a meaningful difference to client outcomes while being backed by a supportive, values‑led organisation with over 25 years of trusted service.
Key Responsibilities:
- Conduct assessments and develop individualised Care Plans and budgets.
- Coordinate multidisciplinary services and monitor delivery against Care Plans.
- Deliver nursing care within scope (RN/EN) and provide education to care workers.
- Advocate for clients, connecting them with health and community services.
- Build referral networks and contribute to Kinder Caring's growth.

Essential Requirements
- Current AHPRA registration (RN or EN) with demonstrated nursing and case management experience in aged care (community preferred).
- Strong knowledge of the Support at Home Program, Aged Care Quality Standards, and experience supporting private clients with tailored, consumer-directed care plans.
- Excellent assessment, care planning, coordination, and communication skills — both written and verbal.
- Ability to work independently with professionalism, empathy, and integrity, maintaining client trust and confidentiality.
- Minimum 15 hours per week availability with flexibility to meet client needs
- Unrestricted work rights and ability to operate as an independent contractor (ABN required)
