About the Role:
We are looking for a Home Care Package Coordinator to join our dedicated Home Care Team in our beautiful Gosnells Village. If you are seeking a new challenge where you can utilise your RN/EN skills, strong coordination ability and aged care industry experience we want to hear from you.
You will coordinate, manage, and optimise client packages, and work to ensure the best care is provided to clients, in keeping with the values of . As the successful applicant, you will be an RN/EN with a sound knowledge and understanding of home care services, and aged care standards. You will be a team player with the ability to work positively and collaboratively with others.
Key Responsibilities:
- Undertake case management including complex, high, and moderate care requirements.
- Conduct clinical assessments and provide holistic, culturally sensitive care to older Australians living in their homes.
- Provide wound care, medication management, chronic disease management, and other clinical tasks relevant to the home environment.
- Support clients in navigation of financial and resource management to ensure needs are met and properly resourced. Develop care plans for the provision of quality person centered support according to client needs, contractual obligations, relevant legislation and policies and procedures.
- Monitor funding and expenditure to maintain services within budget constraints.
- Complete commencement and exit documentation to ensure compliance with policies and procedures.
- Conduct workplace assessments in accordance with WHS guidelines to identify hazards and ensure there is the correct equipment to ensure a safe work environment.
- Liaise with General Practitioners, Allied Health professionals (i.e. physiotherapist, podiatrist, occupational therapist), nursing services, and community organisations to ensure clients' needs are being met.
- Review and monitor compliance with policies and procedures, WHS, Department of Health contractual and legislative obligations.
The Person
- Demonstrated experience in a similar role.
- AHPRA registration as RN or EN.
- High level verbal and written communication skills, with ability to develop relationships and resolve client and staff issues.
- Demonstrated decision making and problem-solving skills.
- Knowledge and understanding of the Aged Care standards, reablement approach to care and the quality improvement process.
- Demonstrated time management and organisational skills.
- Current Police clearance, Flu Vaccination and 'C' Class Driving Licence.
- Previous aged care experience, preferably in a community setting.
- Current National Police Clearance.
