About the role:

At 360 Health Management, we believe in empowering people to live their best lives. As a Specialist Support Coordinator, you'll work closely with participants who have complex needs (Level 2 and 3 NDIS participants) — helping them navigate services, achieve their goals, and build capacity for independence.

Key responsibilities:

  • Coordinate supports for complex NDIS participants with care, consistency, and integrity
  • Build strong relationships with participants, families, carers, and providers
  • Use a coaching and strengths-based approach to build capacity and confidence
  • Manage budgets, reports, and service agreements with precision and transparency
  • Facilitate Care Team meetings and liaise with health professionals and stakeholders
  • Provide crisis response and advocacy as needed, ensuring no one falls through the cracks

Essential Criteria

  • Degree in Social Work, Psychology, Occupational Therapy, Speech Therapy or related field
  • Experience supporting complex NDIS participants in a coordination or allied health capacity
  • Excellent communication and relationship-building skill
  • Strong organisational and time management ability
  • Understanding of NDIS frameworks, service delivery, and ethical practice
  • A current NDIS Worker Screening Check, WWCC, and Driver's Licence
