About the Role

The incumbent for the position of Business Development and Case Manager will be an integral member of our team. This role is pivotal to GNA Services meeting operational and strategic objectives.

Your duties and responsibilities will be divided between business development and case management/coordination of client supports.

Business Development:


• Development and review of a robust marketing plan that aligns with the organisation's purpose, mission, values, and strategic plan.


• Actively promote GNA Services' service offerings, brand, and values through professional engagement, networking, and community outreach like expos and networking events


• Consolidate on existing referral networks such as LACs, hospitals, Support Coordinators, and healthcare professionals.


• Identify and purse new business opportunities across referral networks, partnerships, events, and community platforms, and help grow the GNA Services brand.


• Build and maintain strong relationships with prospective and existing clients, their families, and key external stakeholders.


• Conduct initial consultations with prospective clients to understand their service needs and successfully convert leads into active clients.


• Oversee and facilitate the client onboarding process, ensuring a seamless transition from

enquiry to active service.


• Meet and exceed business growth KPIs through proactive generation of leads and

efficient conversion strategies.

Case Management:


• Oversee and coordinate services for a caseload a clients.


• Develop, implement, and review individualised support/care plans in partnership with cli-ents and their support networks.


• Ensure a safe environment for clients and workers alike through the completion of risk

assessments and associated documentation.


• Lead, mentor, and support a team of Support Workers.


• Build strong, trusting relationships with clients, families, and allied health professionals


• Ensure services comply with NDIS practice standards and organisational policies and proce-dures.


• Manage incidents, complaints and feedback as per compliance/reporting requirements of funding bodies.


• Proactively contribute to service planning and continuous improvement initiatives.

About You

The successful applicant will meet the following essential criteria:


• Experience in both a business development or intake-focused role, and case coordination, ideally within human services such as Disability or Aged Care sectors.


• Minimum 3+ years of relevant experience required.


• Strong understanding of NDIS Practice Standards and/or Aged Care Quality Standards.


• Established and/or the ability to build strong relationships with clients, case managers and other key stakeholders


• Exceptional communication, negotiation, and relationship-building skills.


• Demonstrated initiative, sound judgment, and ability to work autonomously.


• Experience using CRM or care management systems.


• Eligible to work in Australia with valid Australian working rights.


• Current full WA driver's license, own vehicle with comprehensive insurance, and

willingness to travel as needed.

Desirable Criteria


• Previous experience transitioning NDIS participants from hospital to community-based services.


• Experience with Brevity Care software.
