Position Title: Case Manager - Homelessness Specialist Support Service (HSSS)
Department: Tenancy Support Program
Reports To: Program Coordinator – HSSS
Direct Reports: Nil
Key Internal Working Relationships:Chief Executive Officer, Deputy Chief Executive Officer, Team Lead Tenancy Support, Program Coordinator – HSSS, Tenancy Support Team and Southern Aboriginal Corporation (SAC) Employees
Key External Working Relationships:Service providers, stakeholders and community across the Central Great Southern
Location: Katanning, with travel across the Central Great Southern including to the Shires of Kojonup, Gnowangerup and Broomehill-Tambellup

30 hours per week.
Duration (Term) of contract:
Fixed term ends 30 June 2029.
This position is supported by grant funding, and as such, it is contingent upon the grant's continuation. Should the grant funding be reduced or terminated, the position may be subject to change, reduction in hours, or termination.
Wages:
In line with Level 4 of the Social, Community, Home Care & Disability Services Industry Award 2010, with the option of salary sacrifice. Superannuation guarantee paid as legislated (currently 11.5%).

HSSS will provide wrap-around case management support for sixteen (16) clients/families at any one time experiencing or at risk of homelessness in the Central Great Southern Region, including the Shires of Katanning, Kojonup, Gnowangerup and Broomehill-Tambellup. The HSSS Case Manager will work with all persons experiencing or at risk of homelessness and support them to obtain, sustain and/or maintain appropriate accommodation.
About YouYou are a motivated and committed person that essentially has:
- Formal qualification or equivalent expertise in social and community services;
- Demonstrated experience in the development of case plan's structured to achieve identified goals;
- Demonstrated use of organisational processes and systems to ensure that all documentation requirements are completed in a professional and timely manner;
- Demonstrated awareness and knowledge of working with the target cohorts and broad knowledge of issues affecting people from these communities;
- Demonstrated knowledge of the intersection between homelessness, family and domestic violence and mental health and alcohol and other drug use;
- Demonstrated ability to liaise, consult and form cooperative and purposeful relationships with staff members, community, government agencies, service providers, ACCOs and other stakeholders; and
- Highly developed interpersonal, communication and negotiation skills and the capacity to build and maintain relationships with a diverse range of people.
Ideally you will have:
- Experience in similar case management, assisting people experiencing or at risk of homelessness and familiarity with Housing First approach. (highly desirable).
In addition (Mandatory)
- A Current WA ''C' driver's licence.
You must be prepared to undergo:
- A Prior Offender History Check.
- A Working with Children Check.
