Atticus Health is seeking a compassionate and motivated Care Coordinator (Registered Nurse) to join our team. This unique role combines hands-on nursing with care management, supporting older adults to live healthier, more meaningful lives.

About the Role

  • Location: Based in our Hardware Lane clinic (CBD) with outreach across the city and surrounding suburbs.
  • Split Role: 50% clinical nursing within the clinic, 50% care coordination in the community.
  • Focus: Partnering with people over 50, both in clinic and at their homes, to improve their quality of life.

Key Responsibilities

  • Provide nursing support, including clinical assessments and care delivery in the Hardware Lane clinic.
  • Visit patients in their homes, building trusted relationships and supporting them to live well and independently.
  • Act as a Care Manager for patients aged 50+, coordinating care plans and services tailored to individual needs.
  • Collaborate with GPs, allied health, and community providers to ensure seamless, person-centred care.
  • Support patients in navigating the health system, connecting them with relevant services and resources.
  • Maintain accurate records and documentation in line with professional standards.

About You

  • Registered Nurse (AHPRA) – Essential.
  • A genuine "people person" – warm, empathetic and able to connect meaningfully with people over 50.
  • Strong interest in holistic, person-centred care and improving life quality for older Australians.
  • Excellent organisational and communication skills.
  • Current driver's licence and willingness to travel locally to patients' homes.
