Weekend Care Coordinator

Overview

Join the Weekend Care Coordinator & Nurse Mentor role at Infinite Aged Care, a purpose-built aged care home in Knoxfield, Victoria. We are seeking weekend morning (06:30–14:30) and afternoon (14:00–22:00) shifts for Clinical Care Coordinators and Nurse Mentors. Additional Registered Nurse shifts are available, offering opportunities for dual roles. These positions may lead to full-time Clinical Care Coordinator and Nurse Mentor roles in early 2025.

The Role

Join the Knoxfield Clinical Care team in a rewarding and fast-paced role as a Care Coordinator and Nurse Mentor. This full-time role reports to the General Manager, with support from the Clinical Care Manager while supervising and supporting our care team. The key purpose is to ensure all residents receive the highest quality clinical care. The Clinical Care Coordinator manages and resources care needs within the facility, ensuring documentation and assessments are up to date and aligned with best-in-class clinical practices.

Key Responsibilities

• Provide clinical leadership for clinical and care service teams across the Care Home.
• Assist the Clinical Care Manager to lead care teams to meet all resident clinical needs, including monitoring and tracking indicators to achieve best-practice outcomes.
• Manage assessment, documentation and submission of AN-ACC requirements within the facility.
• Support documentation, policies, procedures, and IT system requirements within the facility.
• Coordinate assessment, care planning and review processes within the facility.
• Support admission of new residents with initial assessments and consultations with residents and families.
• Ensure accurate information recording in line with organisational and legislative requirements (AN-ACC, resident records, staff time recording).
• Input and oversee facility rosters to ensure optimal skill mix for resident care.
• Manage and document clinical risk and drive continuous improvement within the facility.

You will offer

• Innate empathy for residents and commitment to continually improving clinical care quality.
• Minimum 1 year experience as a Care Coordinator or similar leadership role.
• Current unrestricted Registered Nurse Div 1 with AHPRA.
• Postgraduate qualifications in aged care are advantageous but not essential.
• Experience and knowledge of aged care clinical assessments, care plans, AN-ACC and accreditation documentation.
• Ability to influence and develop clinical leadership across the team in an aged care environment.
• High level computing skills to maintain documentation and reporting in line with requirements.
• Strong communication and interpersonal skills with a collaborative, team-oriented approach.
• Ability to manage multiple stakeholders with composure in emergencies.
• Valid NDIS Worker Screening Clearance (or the ability to apply).
