Integrated with the St Vincent's hospital and community care network we are a multidisciplinary team of physicians, allied health, nursing and personal care workers supporting individuals to leave hospital sooner, remain in their homes and achieve their goals. We are an experienced, supportive and dynamic team committed to the growth of quality care within the community.**About the Role**We are looking for an enthusiastic Care Coordinator (Registered Nurse or Allied Health Professional) with broad experience in the health care sector to join our Rehabilitation at Home team.
