We are hiring: Team Coordinator – NDIS Services

Position: Team Coordinator (Disability & Mental Health Supports)

About the Role:

We are seeking a proactive and compassionate Team Coordinator to join our NDIS service team. This hybrid role combines hands-on support work with leadership and coordination duties. You will support participants to achieve their individual goals while also supervising a team of support workers, ensuring high-quality service delivery that aligns with NDIS standards. The Role is divided into 2 components:

Key Responsibilities:

Support Work Component:

  • Provide person-centred, strengths-based support to NDIS participants in their homes and the community.
  • Assist participants with daily living activities, personal care, community access, and skill development.
  • Foster participant independence, wellbeing, and community engagement.
  • Maintain accurate documentation, progress notes, and incident reports.

Team Coordination Component:

  • Lead, support, and mentor a team of support workers.
  • Assist in scheduling shifts, managing rosters, and covering shifts when needed.
  • Ensure compliance with participant plans, risk assessments, and behaviour support plans.
  • Monitor service quality, staff performance, and client satisfaction.
  • Support onboarding, training, and supervision of new staff.
  • Collaborate with participants, families, guardians, and other stakeholders.

Role Criteria:

  • Certificate III or higher in Mental Health, Disability, or a related field is desired but not essential.
  • Experience in providing support to people with mental health or disability needs is highly desirable.
  • Experience with behaviour support and complex needs is desirable
  • Strong communication skills and the ability to work collaboratively with participants, families, and other professionals.
  • Proven experience or capacity to lead and coordinate a team.
  • Strong knowledge of the NDIS framework and person-centred practice.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to manage priorities and adapt to changing situations.
  • Proficiency in using client management systems.
