About the role

This Care Coordinator role, internally known as Participant Program Coordinator, you'll play a central role in making sure participant care plans are translated into safe, reliable, and person-centred supports.

You'll work closely with participants, families, support workers, and clinical staff to:

  • Coordinate and monitor care plans
  • Liaise with rostering to ensure the right worker is matched to the right participant at the right time
  • Induct, supervise and lead support workers to deliver the best in person centred care
  • Conduct regular reviews and capture participant outcomes
  • Support incident management and compliance with Best Practice Standards

About you

We're looking for someone with:

  • Experience in disability, aged care or health — ideally in coordination, case management, or scheduling support
  • Strong communication and relationship skills
  • Organisational ability and confidence with systems like Visual Care
  • A balance of empathy and professionalism
