Specialist Support Solutions are an Indigenous NDIS Registered Company leading the change in Service Provider's in the NDIS, Focused on closing the gap between provider and client.

Specialist Support Solutions are seeking an enthusiastic, Experienced Level 2 & 3 Support Coordinator to assist clients navigate the complexities of NDIS, to ensure that their plans are utilized effectively. Specialist Support Solutions is based in Rockhampton QLD, however we are open to hiring Support Coordinators QLD Wide

Desired Requirements:

  • Minimum 2 years experience in Support Coordination
  • A strong drive to contribute to positively impact the lives of our clients, and always go the extra mile
  • Excellent communication skills and interpersonal skills
  • A track record of success reaching productivity and activity targets
  • Strong problem solving and critical thinking skills to address clients needs
  • Ability to complete plan budgets and breakdown of NDIS plan
  • Understanding of the NDIS Code of Conduct
  • Experience in liaising with Allied Health Professionals, Stakeholders, Mental Health, Medical Professionals, Public Guardian and QCAT
  • Relevant diploma/Tertiary qualification in a human services or allied health discipline is advantageous.

Your key responsibilities are:

  • Empower clients with a deeper understanding of their plans and how to implement them
  • Maintaining detailed case notes and administrative records to support NDIS reporting requirements
  • Providing pre and post plan recommendations to assist clients utilize their NDIS plan
  • Coordinate and build strong relationships with internal and external stakeholders
  • Successfully meeting productivity and company targets
  • Implement and Monitor budgets and support to ensure that funding expenditure is on track to the desired goal
  • Conduct regular progress reviews of client's plan, ensuring timely and accurate reporting to the NDIS
